Penoscrotal approach surgery performed by Dr. Jay Simhan. The surgery consists of Penoscrotal incision with anticipation of scrotoplasty, Identification of proximal corporal body with corporotomy and hydrodilation, Placement of Furlow for sizing measurement of the corporal body, Placement of the penile implant reservoir through a submuscular/ectopic approach with a long lung grasping forcep, Placement of penile implant cylinders with pump connection to reservoir and Pump placement in the dependent scrotum.

Technique Description and Procedure Steps:

  • 1. Penoscrotal incision with anticipation of scrotoplasty
  • 2. Identification of proximal corporal body with corporotomy and hydrodilation
  • 3. Placement of Furlow for sizing measurement of the corporal body
  • 4. Placement of the penile implant reservoir through a submuscular/ectopic approach with a long lung grasping forcep
  • 5. Placement of penile implant cylinders with pump connection to reservoir
  • 6. Pump placement in the dependent scrotum.

Clinical History:

  • 58 year old male with a history of refractory erectile dysfunction
  • Patient has failed prior conventional treatments for ED including PDE5 therapy and penile injection therapy.
  • Patient decided to undergo penile implant surgery through penoscrotal approach for treatment of refractory ED.
