A 68 years-old female presenting with anal squamous cell carcinoma T3N0M0 2 years ago, treated with chemoradiation (CDDP-FU and 45 Gy). On the follow-up imaging, a 10 x 7 mm lymph node is localized on the mesorectum, 10 cm from the anal verge. PAAF confirms metastasis of squamous carcinoma. No anal or rectal lesions are found. No other lesions are found on the MRI, CT scan or PET.
How would you approach lymph node recurrence for anal cancer?
